Waiver, Release of Liability, and Indemnification Agreement
In consideration of being permitted to participate in Camp Vincent (also known as Gabe Vincent's Annual Basketball Skills Camp) (the “Camp”) hosted by Just Watch Sports LLC (“JWS”) at Santa Barbara High School on August 29, 2026, the undersigned parent or legal guardian (the “Parent”) of the participating minor athlete (the “Athlete”) agrees to the following terms:
1. Assumption of Risk
Parent understands that participation in basketball and related athletic activities involves inherent risks of injury, including but not limited to bumps, bruises, sprains, strains, fractures, concussions, dental injuries, and in rare cases more serious injury or death. Parent voluntarily accepts these risks on behalf of the Athlete.
2. Release of Liability
Parent, on behalf of themselves and the Athlete, releases and discharges Just Watch Sports LLC, its officers, directors, employees, coaches, volunteers, and agents, as well as Santa Barbara High School, the Santa Barbara Unified School District, and their respective officers, employees, and agents (collectively, the “Released Parties”), from any and all claims, demands, damages, or causes of action arising from the Athlete's participation in the Camp — except those caused by gross negligence or willful misconduct.
3. Medical Authorization
Parent authorizes Camp staff to administer basic first aid as needed. In the event of a medical emergency where Parent cannot be reached, Parent authorizes Camp staff to seek emergency medical treatment for the Athlete from qualified medical personnel. Parent agrees to be responsible for all costs associated with medical treatment beyond what is covered by the Camp's secondary medical insurance.
4. Insurance
The Camp maintains general liability insurance and participant medical insurance through AIG Specialty Insurance Company (Policy #9YAPG0001334486302). Parent understands this coverage is SECONDARY to the family's own health insurance. Parent confirms that the Athlete is covered by primary medical insurance and that Parent will provide insurance information upon request.
5. Accuracy of Medical Information
Parent confirms that the medical, allergy, and dietary information provided in the registration form is accurate and complete to the best of their knowledge. Parent will promptly notify the Camp of any changes prior to the Camp date. The Camp may decline participation if undisclosed medical conditions are discovered that may put the Athlete or others at risk.
6. Photo and Video Release
Parent grants Just Watch Sports LLC permission to photograph and video the Athlete during Camp activities, and to use these materials for promotional, marketing, educational, and editorial purposes in print, digital, and broadcast media without compensation. Parent may request, in writing prior to the Camp, that the Athlete not be photographed; in such cases, JWS will use reasonable efforts to accommodate.
7. Code of Conduct
Parent agrees that the Athlete will follow Camp rules and behave respectfully toward coaches, staff, and other participants. The Camp reserves the right to remove any athlete whose conduct is disruptive, dangerous, or significantly disrespectful, without refund. Parents will be contacted immediately if removal is necessary.
8. Refund Policy
Camp registrations are eligible for refund under the following terms:
- Cancellations 30 or more days before camp (on or before July 30, 2026): 75% refund (covering payment processing and administrative costs)
- Cancellations less than 30 days before camp: no refund
- No-shows on camp day: no refund
- If the Camp is cancelled by JWS for any reason: 100% refund
The 3% processing fee is non-refundable in all cases, as it reflects costs already incurred from the payment processor.
9. Indemnification
Parent agrees to indemnify, defend, and hold harmless the Released Parties from any and all claims, damages, losses, and reasonable attorney's fees arising from the Athlete's conduct or participation in the Camp, except those caused by the gross negligence or willful misconduct of a Released Party.
10. Governing Law and Dispute Resolution
This agreement is governed by the laws of the State of California. Any dispute arising from this agreement that cannot be resolved through good-faith negotiation shall be brought in the state or federal courts located in Sacramento County, California.
11. Severability
If any provision of this agreement is held to be unenforceable by a court of competent jurisdiction, the remaining provisions shall remain in full force and effect.
12. Entire Agreement
This agreement constitutes the entire agreement between Parent and JWS regarding the Athlete's participation in the Camp and supersedes all prior or contemporaneous agreements.
